Comments: Hepatitis A shot record is incomplete; reach-in cooler has no thermometer; damp cloth used for stabilizing a cutting board; walk-in fan guards have build up of dirt/dust.

Comments: Damp mop not hung to air dry in-between uses; interior surfaces of ice machine have a build-up of black mold-like substance.

Comments: No violations seen during inspection.

Comments: Eggs stored above other foods; no thermometer found in reach-in cooler; walk-in cooler’s gasket has heavy build up of black oily substance.

Longhorn Steakhouse
Address: 2317 North Highway 67, Florissant

Comments: Customer complained about being ill after eating at the restaurant. She ate a salad, grilled chicken breast with mushrooms, macaroni and cheese. Inspector spoke to the management who showed temperatures in acceptable ranges, and chicken and produce delivered fresh throughout the week and macaroni and cheese is frozen then thawed. No other complaints received for other foods on that date.

Papa John’s Pizza
Address: 12735 New Halls Ferry Road, Florissant
Date of Inspection: Aug. 5
Type of Inspection: Routine
Risk Factor: 2
Grade: 97-A
Comments: Inspector saw pest flies and fruit flies in kitchen area and need to be treated before follow-up inspection; toppings in cooler were not dated but later corrected; incomplete Hepatitis A records provided but later corrected; peppers had no cover but later corrected.
*Follow-up inspection scheduled for Aug. 15.

2 Girls 4 Wheels
Address: 21 Village Square Shopping Center, Hazelwood
Date of Inspection: July 19
Type of Inspection: Opening

Grade: 98-A
Comments: Roof hatch is torn and Hepatitis A records needed. Inspector noted the commissary is Wave Taco at 1335 Convention Plaza. It’s been approved to open July 22.

Chevy’s Fresh Mex
Address: 5555 St. Louis Mills Blvd., Hazelwood
Date of Inspection: July 8

Grade: 90-A
Comments: Inspector told employee to wash hand before putting on gloves and in-between tasks; inspector noticed tomatoes with mold growth and insects in storage area; food debris found inside utensil and lid containers; no proof of Hepatitis A records; no thermometer available to measure temperatures of coolers; drinking straws not properly handled to protect food and lip contact.
