Interior surfaces (ceiling) of the ice machine have a build up of a black mold like substance. Reach in cooler door gasket has a build up of old food particles. Damp mop not hung to air dry when not in active use. Freezer floor has a build up of various debris.

Cardboard under food in walk in cooler. Debris build up in veggie cooler. Backflow prevention device not provided, approved, or properly located. Light fixture is missing light bulb shield.
