Exhaust hood filters are heavily-soiled; floors are heavily-soiled under all cooking equipment; the main food preparation table shelves are soiled, top surface of the woks are soiled,  sides of all cooking equipment are soiled, top surface of oven range is soiled, top surface of exhaust hood is soiled. clean all areas. Employee is wearing heavily-soiled pants. Replace clothing and wash. Monosodium glutamate container on the wok line is not labeled with contents. Label containers. Cooked foods are being placed in raw poultry box tops and there is condensate from the compressor dripping onto food inside the walk-in cooler. Relocate food. The kitchen hand-washing sink is blocked. Provide access and relocate equipment.”

Inspector Comments: “Complainant stated that she ate a jumbo jack and got severely ill with stomach cramps, diarrhea, and vomiting. She went to the emergency room on 6/10 and they diagnosed as food poisoning. Spoke with manager, Kara Chisholm, and she stated that the jumbo jack sandwich is made with or without cheese, mayonnaise, ketchup, onions, lettuce, tomatoes, and pickles on a seeded bun. The hamburger patties are frozen and cooked for 2 minutes and 10 seconds under a presser, and for 5 minutes and 40 seconds without a presser. If the hamburgers are not cooked to order, they are cooked and held for 20 minutes and discarded if not used. The onions are pre-packaged and pre-sliced and held for 4 hours. The pickles are pre-sliced and held for 30 days. Tomatoes are cut in-house and held for 2 hours. The head lettuce is cut with a slicer and held for 2 hours. Observed employees washing hands before putting on gloves to handle food and between tasks. No violations observed. No follow-up required.”
