Comments: Hepatitis A documentation not maintained in a clear and concise manner. Observed damaged and missing tiles throughout the kitchen. Inspector advised that all damaged tiles and cove basing shall be repaired as needed. There is a missing ceiling tile in the utility closet that will also be replaced.

Comments: Sink has leak from faucet. Vent in mop sink area has debris. An open beverage cup (one without a lid and a straw) is in the kitchen preparation area.

Comments: Raw Chicken over veggies and cooked foods WIC and WIF Foods not covered in walk in freezer and cooler. Chicken & ham in walk in cooler reading at 56. Wall damage at sink area, needs new caulk. Debris build up under sink. Kick plate on walk in freezer broken. Need labels on food at bar area. Follow up inspection scheduled December 6.
